Visual Outcomes Of Cataract Surgery In Patients After Presbyond Laser Blended Vision

Purpose

To report the visual outcomes of cataract surgery in eyes that have previously undergone refractive correction with PRESBYOND Laser Blended Vision LASIK.

London Vision Clinic, London, UK

Methods

This was a retrospective analysis of 156 eyes that underwent cataract surgery after having previously undergone PRESBYOND Laser Blended Vision LASIK between January 2015 and May 2023. The corneal spherical aberration was measured and an appropriate monofocal intraocular lens was chosen in order to optimize the postoperative depth of field afforded by the primary PRESBYOND procedure. IOL models used included Alcon SN60WF, Rayner RayOne, Rayner EMV. The Haigis-L formula was used from 2015-2018 and the Barrett True K formula was used from 2019-2023. Standard outcomes analysis was performed using the data at 12 months.

Results

Mean attempted SEQ was -0.80±2.04 D (-8.00 to +2.88 D). Mean cylinder was -0.82±0.63 D (0.00 to -3.25 D). Corneal astigmatism was 0.80±0.51 D (0.00 to 3.52 D). Following cataract surgery, CDVA was 20/20 or better in 81%, 20/25 or better in 92% and 20/40 or better in 99%. Binocular UDVA was 20/20 or better in 65%, 20/25 or better in 84% and 20/40 or better in 98%. Binocular UIVA was 20/32 or better in 64% and 20/63 or better in 95%. Binocular UNVA was J2 or better in 69% and J5 or better in 92%. Postoperative SEQ was within ±0.50 D in 74% and ±1.00 D in 92%. Postoperative cylinder was within 0.50 D in 63% and 1.00 D in 93%. Contrast sensitivity was within 1 patch of normal in 95% eyes at 3 cpd, 92% at 6 cpd, 77% at 12 cpd and 70% at 18 cpd.

Conclusions

Monofocal cataract surgery after previous PRESBYOND Laser Blended Vision LASIK using standard post-LASIK IOL calculation formulae provides similar efficacy and accuracy to primary cataract surgery. Utilizing the existing corneal spherical aberration to select the appropriate monofocal IOL asphericity provides good uncorrected vision for distance, intermediate and near with less anisometropia than needed for standard monovision. The use of monofocal IOLs provides 100% light transmission, absence of diffractive IOL side-effects and the expected low PCO rate.
