Visual Outcomes Of Continuous Power Enhanced Monofocal Lenses Targeting Blended Vision For Presbyopia Management

Purpose
The Tecnis Eyhance IOL is an enhanced monofocal lens offering distance to intermediate correction when targeting plano. The goal of this study is to evaluate clinical outcomes and subjective visual function following implantation of a Tecnis Eyhance IOL in blended vision for patients with presbyopia.
Setting
Retrospective-prospective chart review at a single site in Ottawa, ON.
Methods
This is a retrospective-prospective, non-randomized, single-centre study of patients undergoing cataract or refractive lens exchange with implantation of the Tecnis Eyhance IOL between August 2020 to December 2022. The procedure aimed for blended vision with a target of emmetropia in the dominant eye and a target of myopia ranging from -0.75 to -3.0 D in the non-dominant eye. Patients were divided into three groups: Group 1 - Plano OU (+0.5 to -0.5 D) (n = 16), Group 2 = Blended (-0.75 to -1.25 D) (n = 10), Group 3 - Blended (-1.26 to -3.0 D) (n = 15). Key outcomes included 3-month postoperative uncorrected visual acuities at distance, intermediate, and near, defocus curves, spectacle independence, and NEI-VFQ questionnaire visual function.
Results
Forty-one participants (82 eyes) were evaluated. Mean binocular uncorrected distance, intermediate, and near visual acuities at three months in Group 2 were -0.03, 0.12, and 0.27 logMAR, respectively (p > 0.05) and -0.07, 0.03, and 0.21 logMAR (p < 0.05 for near vision) in Group 3. A defocus of -4.00 D to -1.50 D demonstrated an improved binocular VA in Group 3, corresponding to a near and intermediate area of focus and reading distance of approximately 25 to 66 cm (p < 0.05). Group 3 showed a greater difference in spectacle independence than Group 1 for both intermediate and near vision (p < 0.05). The mean NEI-VFQ questionnaire score at three-months was 78.3, 90.0, 91.7 in Group 2, compared to 92.5, 92.1, and 92.5 in Group 3 (p < 0.05).
Conclusions
The Tecnis Eyhance continuous power IOL provides acceptable distance vision and excellent intermediate and near vision when implanted targeting blended vision from > -1.26 D. Accordingly, it improves quality of life and allows spectacle independence for most distance and intermediate activities.
