Video Summary

The capsulorhexis has a tendency to run-out if the anterior lens capsule is insufficiently flattened, if there is any posterior pressure, and if the anterior chamber shallows.

The trouble starts when the anterior chamber shallows due to loss of viscoelastic. This happened when the forceps were distorting the incision. The anterior lens capsule reverts back to its curved or domed contour which allows the capsulorhexis to run downhill and out towards the zonular attachments.

This simple procedure allows cataract surgeons to rescue a capsulorhexis which is running out toward the lens equator. By pulling backward, the tear can be brought back centrally and then the capsulorhexis continued in a safe manner.
