Video Summary
I present a case of cataract surgery in a patient with soft cataract.
The surgery began without incidents. However, when the phacoemulsification phase arrives, then it gets complicated. An attempt is made to make the main groove, but since it is difficult to split the cataract, it is decided to linearly phacoemulsify the superficial part of the cataract and try, through aspiration, to dislocate the cataract so that it can be aspirated in the anterior chamber without incident. However, during the process, a temporary zonular disinsertion is generated, so viscoelastic is quickly introduced to prevent the passage of vitreous into the anterior chamber and by means of a capsular tension ring the sac is stabilized. Subsequently, to avoid removing the cataract suddenly, it is decided to use the irrigator-aspirator to finish suctioning the cataract. The masses are aspirated and finally a bag lens is inserted without incident.
