Video Summary

This is the case of a patient with a dense white cataract with a fibrotic and calcified plaque.

After doing our paracenthesis, blue dye stain and viscoelastic fill of the eye, we use a sharp insuline needle to puncture the anterior lens capsule. Forceps can now be used to tear the more normal peripheral capsule where we stay away from the plaque until a fibrous band is reached.

At this point, there is resistance to capsulorhexis creation. Forceps are used for countertraction to pull the capsule flap in the correct direction while we try to keep the fibrotic plaque inside the margins of our capsulorhexis.

With a normal sized capsulorhexis, we can start phaco emulsification with a stop and chop technique.

As our elders always told us, one shouldn’t think it’s over until it’s over:

After making our trench, the cracking we do doesn't propagate well to the posterior plate, which is gummy and looks fibrotic too.We use our sinskey hook with a second instrument to break apart the posterior plate.
