Video Summary

A resident is operating a young myopic (-6 D SE) woman (aged 57) with a corticonuclear cataract. At the beginning of the surgery phacodonesis is seen as shown in the video. A 2-3 clock hours inferonasl zonular dyalisis is seen. The surgeon prefer to continue the surgery without the insertion of iris hooks to stabilize the rhexis. After gentle hydrodissection and hydrodelineation, phacoemulsification with a stop and vertical chop technique is carried out. Important is to work always with the anterior chamber filled with BSS-viscoelastic avoiding anterior chamber collapse. At the end prior of the cortex removal a capsular tension ring is inserted. A dibate is when introducing the ring a the IOL in this cases. The earlier the more stable the bag but the more difficult the cortex removal. We decide to insert immediately the capsular tension ring due to bag instability and also beacuse of not much cortex remanents. The surgery is completed withouth complications and in a reasonable time.
