Purpose
to study of the effectiveness of the laser cyclocoagulation method (using the Lika-Surgeon diode laser) in patients with secondary neovascular glaucoma
Setting
An alternative to enucleation in neovascular glaucoma are methods to reduce the secretion of intraocular fluid, which is achieved by using a diode laser.
Methods
There were 18 patients under observation (18 eyes), the average age was 70=3.0 years. The criteria for selecting patients were: pronounced pain syndrome; neovascularization of the iris; keratopathy; high IOP.
8 patients had postthrombotic glaucoma, and 10 had neovascular diabetic glaucoma.
The average IOP level was 40.2 mm Hg, visual acuity - from 0 to light perception.
All patients underwent laser cyclocoagulation. In the postoperative period, instillations of corticosteroids, non-steroidal anti-inflammatory and hypotensive drugs were received.
Observation lasted 6 months.
Results
In the postoperative period, reactive inflammatory syndrome (moderate increase in IOP and pain of the eyeball) was present in 4 eyes. In 1 eye, there was a pronounced inflammatory reaction with precipitates on the back surface of the cornea and fibrin in the anterior chamber. After 10 days, the patients' pain and redness disappeared and IOP normalized. In only one patient, the inflammatory reaction lasted more than 2 weeks, and hypotonia was observed later.
Conclusion
The use of the laser cyclocoagulation method is effective and allows you to reduce IOP, relieve pain and save the eye in patients with secondary neovascular glaucoma.
