Purpose

Conjunctival granulomas are rare entities that can occur in various contexts, post-surgical, infectious or foreign body (FB).

We report the case of a 10-year-old child who developed conjunctival granuloma on foreign body in the right eye following a neglected trauma.

Setting

. Department of Ophthalmology B, Rabat Specialty Hospital, CHU ibn Sina, Mohammed V Souissi University Rabat

Methods

The case involved a 10-year-old child with a 3-week history of neglected thornbone trauma, who consulted the emergency department for a slightly painful red eye with no decrease in visual acuity in the right eye.  Ophthalmological examination revealed ODG visual acuity of 10/10. Examination of the adnexa revealed mild palpebral edema with conjunctival hyperemia, especially in the temporal region, and an irregular whitish conjunctival swelling 2mm*2mm*2mm in size, surrounded by an inflammatory reaction with the presence of a subconjunctival plant spine . Examination of the anterior segment and fundus was unremarkable, while examination of the adelphic eye was normal.

Results

The patient underwent surgical excision of the granuloma with removal of the vegetal spine, and was treated with steroidal anti-inflammatory agents with good clinical improvement.

Conclusion

Conjunctival foreign-body granulomas are probably still underestimated. It is always important to determine whether or not an intraocular CE exists, and this can only be done by careful questioning. Prompt treatment will prevent the onset of complications that can compromise visual prognosis.
