Purpose

Background:

•Although toric IOLs (tIOL) significantly improve postoperative vision by compensating for corneal astigmatism over the course of cataract surgery, postoperative residual astigmatism remains a major concern, with approximately one-third ofpatients not achieving the targeted refraction after tIOL implantation.

•Accurate keratometry measurements (including the axis and the refractive power of the anterior corneal surface) are essential to achieve an optimal refractive result.

•Standard keratometry is based solely on measurement of the anterior corneal surface and therefore does not considercorneal thickness and posterior corneal surface. Nonetheless, these features contribute to the overall thickness of the cornea as well as to the assessment of the astigmatism of the eye. Ignoring the posterior curvature of the cornea can result in under- or over-correction of corneal astigmatism.

Setting

Patient recruitment

Patients who will receive a toric are being aksed to join the study at the presurgical visit prior to cataract surgery. If patients agree measurements will be done at the presurgical visit as well as another visit prior to cataract surgery of the first eye. Another follow-up visit is planned for three months after surgery.

Aim of the study:

•To evaluate and compare 4 different instruments for measuring the anterior corneal keratometry values considering theaxis and strength of the corneal astigmatism.
-IOL Master 700 (Swept Source OCT)
-MS39 (Placido disc combined with spectral domain OCT)
-Sirius (Scheimpflug combined with Placido disc)
-Anterion OCT (Swept Source OCT)

•Variables: Keratometry (K values and axis)

Methods

At the first visits patients receive three measurements with the IOL Master 700 as well as three visits with the Sirius. Additionally, patients undergo visual assessments and general ophthalmological assessment (cataract assessment, corneal assessment, funduscopy). The tIOL is calculated at this visit.

At the next visit patients undergo another three measurements with the IOL Master 700, as well as three measurements with the Anterion OCT as well as the MS39.

At the follow- up visit the position of the toric IOL is measured. Patients undergo visual assessment and a measurement with the IOL Master 700.

Results

Results will be presented at the ESCRS conference. Until now we have recruited 57 eyes, but are planning on finishing the recruitment goal by february 2024 by including 80 eyes.

Results will be represented using Bland- Altmann Plots. The Bland- Altmann Plots show the difference in between the values as well as the average of the compared values.

Conclusion

Will be presented at the conference.
