Purpose

Our results of clear lens extraction and trifocal intraocular lens (IOL) to correct hyperopia between +4.50 and +12.50 D, as well as evaluation of two intraocular lens calculation formulas, Barett II and Haigis.

Setting

Eye Hospital,Pristina,Kosovo

Methods

Thirty-four patients (64 eyes) who underwent ZEISS AT LISA tri 839MP IOL implantation with clear lens extraction were included in this retrospective study. Included patients were aged 21–47 years, desired spectacle independence, and had preoperative hyperopic refraction (range +4.50 to +12.50 D). The refractive goal was -0.50 D, using the Haigis formula in 32 eyes and the Barrett II formula in 32 eyes. Follow-up was up to 2 years.

Results

Mean postoperative uncorrected visual acuity was 0.8 (range 0.6 to 1.0).  Stability of refraction was noted from the first month after surgery. A residual refraction of -1.0 was detected in one eye of a patient for whom the Barrett II formula was used, but no action was taken because the patient was satisfied. However, myopic residual refraction ranging from -1.50 to -2.75 was observed in 8 eyes out of 32 to which the Haigis formula was applied. Residual refraction was treated by performing SMILE surgery in these eight eyes.

Conclusion

Clear lens extraction is a safe, effective procedure for treating hyperopia between +4.50 and +12.50 D. Using the "-0.50 D" rule, the Barrett II formula proved superior to Haigis in intraocular lens calculation. SMILE is an effective and safe method in the treatment of residual refractive after trifocal IOLs in young adults.
