Purpose

To compare the visual performance of advanced monofocal lens with classic monofocal used in a micro monovision setting after bilateral implantation

Setting

Tertiary eye care centre, Saatvik eye hospital

Methods

In study group, we used advanced monofocals Eyecryl-sert PLHFD6 (n = 50) and ALCON Vivity DFT015  (n=50) while we used AcrySof IQ SN60WF (n=50) in control group. Bilateral implantation was done in line with micro monovision principle, keeping the distant dominant eye as emmetropic and near dominant eye as myopic. Performance was assessed by corrected and uncorrected distance visual acuity (CDVA, UCDVA) at 6 m, uncorrected intermediate VA (UCIVA) at 60 cm and uncorrected near VA (UCNVA) at 30 cm

Results

UCDVA values did not differ significantly across all groups when considering only the distant dominant eye (0.1 ± 0.1) across all groups. However the UCDVA was significantly better in the test groups using the advnaced monofocals when averaged for both eyes. The UCDVA was only ( 0.48 ± 0.1) for the monofocals versus ( 0.18 ± 0.1) for the advanced monofocals . The UCIVA  and UCNVA values did not differ significantly across all groups for near dominant eye. However the advanced monofocals outperformed the monofocal for both intermidiate and near vision when averaged for both eye. The UCIVA for the monofocal when averaged for both eyes was just  0.6 ± 0.1 when compared to advanced monofocal ( 0.1 ± 0.1) . Similarly UCNVA for the monofocal when averaged for both eyes was just  0.6 ± 0.1 when compared to advanced monofocal ( 0.2 ± 0.1). No significant difference was seen between the two advanced monofocals. All the visual acuity were converted to logMAR for comparison .

Conclusion

Micro monovision in advanced monofocal offers better continuous range of vision as compared to traditional monofocal monovision, when considering the diatant as well as the near vision. This was true even for the near dominant eye, which had good distant acuity in the advnaced monofocals group inspite of being myopic. Similarly the advanced monofocal offered good near acuity for the distant dominant eye, left emmetropic, due to their extended depth of focus.
