Purpose

Ocular trauma is a frequent reason for consultation in ophthalmology, and as it is most common in young people, it is a public health problem with a serious impact on visual prognosis. The crystalline lens is often affected by
trauma, leading to post-traumatic cataracts (ruptured or not), lens dislocation, phacolytic uveitis or hypertonia of crystalline origin. Treatment is always surgical and functional recovery
functional recovery depends on other associated lesions secondary to the trauma. The aim of our work is to establish an epidemio-clinical aspect of crystalline trauma in our department.

Setting

Retrospective descriptive study, from January 2021 to August 2023, we recorded 220 patients operated on for post-traumatic lens damage.

Methods

Retrospective descriptive study, from January 2021 to August 2023, we recorded 220 patients operated on for post-traumatic lens damage.

Results

220 Patients with post-traumatic lens damage
Age range 18 - 40 years
Male in 85% of cases
Mechanism of trauma: Accident at work +++; Accident at home ++; Assault (Brawl) +.
Unilateral damage = 100%.
Visual acuity: less than 1/10 = 80%, 20% positive light perception
58% Faded cataract
Penetrating trauma 82
Contusive trauma 18
Unruptured cataract 14
89% contusive trauma
Cataract found in 72% of cases
Phacolytic uveitis 22
79% associated with ocular hypertonia
28% lens dislocation; 57 patients = Age > 60 years; 4 patients = Marfan's disease; 84% anterior dislocation; 14% posterior dislocation; 2 cases of subconjunctival dislocation
Ocular hypertonia found in 88% of cases associated with anterior lens dislocation.

Conclusion

Le cristallin est vulnérable aux traumatismes oculaires, qu’ils soient contusifs ou perforants. Ces atteintes peuvent compromettre le pronostic fonctionnel, d’où l’intérêt de la prévention contre ces traumatismes.
