Purpose

Different techniques and materials have been established to facilitate the capsulorhexis during cataract surgery. A controlled creation of the capsulorhexis is a decisive step in the process not often deciding about the outcome.

This study was set up to determine the influence of different surgical approaches on the intraocular pressure (IOP) before, during and after the creation of the capsulorhexis as an indicator of stability.

Setting

Private surgical centre

Methods

Prospective experimental study of IOP during cataract surgery measured before (T2) and after (T3) creation of the capsulorhexis. 261 eyes were included into this study. IOP was measured using the iCare 200 rebound tonometer.

The creation of the capsulorhexis was achieved using an Utrata forceps (group U) and a bent 26G needle (group N).

Results

IOP dropped significantly from T2 to T3 in all cases during the creation of the capsulrhexis.

Group U showed an IOP drop of 67,1mmHg (SD ± 12,3). Using different OVD showed no significant difference.

Group N showed an IOP drop of 56,5mmHg (SD ± 11,6). Using different OVD showed no significant difference.

The difference between the groups was statistically significant (p<0,001).

Conclusion

The use of different surgical instruments produced a significant different IOP drop comparing the Utrata forceps and the 26G canula. During the capsulorhexis a higher IOP could be maintained by using the 26G canula. This can be used advantageously in complicated situations as elevated vitreous pressure or elevated intracapsular pressure.
