Purpose

To describe the use of ZEPTO capsulotomy handpiece (Mynosys Cellular Devices; Fremont, CA, USA) for the treatment of Anterior Capsule Contraction Syndrome (ACCS) after uneventful phacoemulsfication with posterior chamber intraocular lens implant (PC-IOL) in a 41-year-old female patient with retinitis pigmentosa (RP).

Setting

Precise Vision Ophthalmologists, Rheine Eye Clinic, Rheine, Germany

Methods

A 41-year-old female patient with known RP, nystagmus and bilateral nuclear cataracts underwent bilateral sequential uneventful cataract surgery with PC-IOL. Pre-operative best corrected visual acuity (BCVA) was 20/200 in both eyes on Snellen acuity chart. One month after cataract surgery, BCVA improved to 20/80 Snellen in both eyes with a refraction of ‑0.5 /‑1.25 × 20 and ‑0.25/‑1.0 × 135 right (OD) and left eye (OS) respectively. Six months after cataract extraction, the right eye developed ACCS. There was anterior haptic displacement of all four IOL haptics with subsequently reduced visual acuity. BCVA OD was 20/100 Snellen with refraction OD: 0.00/-2.00*180. A secondary capsulotomy was performed using the ZEPTO device for the management of ACCS.

Results

No intraoperative complications occurred. Post-operatively, BCVA OD improved to 20/80 with a refraction OD: ‑0.25/‑1.00*20 and has remained stable until now.

Conclusion

Our case is the first published application of ZEPTO for the management of ACCS post uneventful cataract surgery in a retinitis pigmentosa patient. ZEPTO is a safe and simple technique for the management of post cataract surgery ACCS and it may be considered as an alternative option.
