Purpose

Dry eye is a common condition after phacoemulsification. It is caused by the section of corneal nerves, microscopic light exposure, as well as due to a destruction of goblet cells and corneal epithelial cells during the surgery. The purpose of the study was to investigate an influence of pseudoexfoliation glaucoma (XFG) and primary open angle glaucoma (POAG) on the dry eye development after phacemulsification.

Setting

Clinic of ophthalmology, University clinical center Kragujevac, Serbia

Methods

Prospective study included 64 patients divided into three groups (I=21 XFG, II=20 POAG, III= 23 control group) who underwent uneventful phacoemulsification. None of the had dry eye before phacoemulsification. Patients were examined and analyzed 1,3 and 6 months after phacoemulsification. Shirmer and TBUT tests were used to diagnose dry eye. For statistical analyses χ2 test and paired t-test were used, and p < 0.05 was accepted as statistically significant.

Results

One month after phacoemulsification dry eye was present in all three groups. Three months after phacoemulsification no dry eye was present in control group. TBUT and Schirmer test in 2nd and 3rd group were significantly lower compared to the control group (p<0.05) with a developed dry eye. Six months after phacoemulsification XFG patients still had dry eye, while dry eye tests in POAG and control group were physiological.

Conclusion

According to our results dry eye development is very frequent after cataract surgery during the first postoperative month. In glaucoma patients, especially in XFG, due to usage of antiglaucomatous eye drops, disturbance of goblet cells dry eye can last many months after the cataract surgery. Thus, is very important to prescribe artificial tear in postoperative period for all patients, particularly in patients with pseudoexfoilations.
