Purpose

the aim of our work is to recognize and treat macular edema post-cataract surgery early.

Setting

retrospective study

Methods

We conducted a retrospective study spanning 6 months (June 2022 to December 2022) including 150 eyes in patients operated for cataract by phacoemulsification. All patients received a preoperative and postoperative ophthalmological examination on Day 1, Day 3, Day 7, Day 30 then 3 months and 6 months with retinal angiography and macular OCT in case of suspicion of OM.

Results

- Of the 150 included eyes, 6 eyes (6patients) (4%) had postoperative macular oedema (OM). The mean age was 62 years of which 2 women and 4 men, 4 patients have type 1 diabetes on insulin.

- Among the 6 eyes, 62% presented a capsular rupture during the procedure, two benefited from a secondary implantation; 38% of eyes benefited from a phacoemulsification without incidents.

- Clinically, all eyes had a decreased visual acuity of at least three lines on average eight weeks after surgery with FO OM appearance in all patients.

- Macular OCT was First-line treatment was oral acetazolamide plus eye drops, 2 patients received triamcinolone injections after first-line treatment failure and one patient who had recurrent OM received intravitreal dexamethasone 0.7 mg implant. the reference examination +/- Retinal angiography.

- The course was good in 5 patients( 5eyes) (83.4%) with a reduction in central macular thickness from 526.16 μm to 235.7 ± 25.6 microns. Occurrence of a macular hole in a single patient (1eye).

Conclusion

It is a cause of visual acuity after cataract surgery. Control of preoperative risk factors is essential. Avoid intraoperative complications Macular OCT is the reference examination. The spontaneous evolution is favorable.
