Purpose
To identify the prevalence and risk factors of ocular surface disease (OSD) in patients receiving topical antiglaucoma medication.
Setting
At the ophthalmology outpatient clinic of CHU Tahar Sfar Mahdia in Est-Central Tunisia.
Methods
This was a comparative cross-sectional study of 160 patients including 120 patients in glaucoma group and 40 volunteers in control group. The number of medications used, daily and cumulative preservative concentrations (PC), fluorescein corneal staining score (Oxford scale), noninvasive tear film breakup time (NI-TBUT) and OSD symptom questionnaire index (OSDI) were recorded.
Results
Significantly higher OSDI (>23: 15% vs 0; p=0.023), Oxford scale (>1: 83.3% vs 30%; p<0.001), and NI-BUT (≤10s: 89.2% vs 50%; p<0.001) were noticed in glaucoma patient compared to control group respectively. However, the Schirmer test showed no difference between the two groups. In the glaucoma group, multivariate analysis showed that higher OSDI score was associated with number of instillations per day (odd ratio [OR] 8.83) and number of medication (OR 6.03) and that severe dry eye signs were associated with diabetes (OR 4.97), Daily PC (OR 8.56) and cumulative PC (OR 4.54).
Conclusion
OSD is common among glaucoma patients. The main risk factors of this disease were number of medications, number of instillations per day, daily and cumulative PC and diabetes.
