Purpose

Nonadherence of the membrane is possible during the triple DMEK procedure, which may require a new air or gas endotamponade (re-bubbling). The aim is to detect parameters that have an influence on the occurrence of re-bubbling after the triple DMEK procedure.

Setting

University Frankfurt Department of Ophthalmology, centre for cornea, refractive and cataract surgery

Methods

Analysis of triple DMEK procedures in the period from January 2016 to December 2022 by means of a retrospective case-control study. Review of patient records, device measurements and accompanying documentation of the respective grafts from sending corneal banks.

Results

280 triple DMEK procedures were performed by two surgeons (OP1 n=136; OP2 n=142) between January 2016 and December 2022. Re-bubbling procedures occurred in a total of 69 cases in 64 patients (age (MW ±SD): 66.34 ± 8.57 years; women vs. men: n= 151 vs. 129; site: OD vs. OS: n= 146 vs. 134; surgeon OP1 n = 36 vs. surgeon OP2 n = 33). Statistically significant factors for the occurrence of re-bubbling due to ocular biometry or endothelial tissue in the triple DMEK procedure were not found in a logistic regression, but a tendency towards axial length (p=0.09). An effect on the re-bubbling rate was also not found by surgeon selection after analysis.

Conclusion

No significant factors influencing the re-bubbling rate after a triple DMEK procedure were observed.
