Purpose
To evaluate the outcomes of gonioscopy-assisted transluminal trabeculotomy (GATT) and trabeculectomy with mitomycin-C (Trab-MMC).
Setting
Retrospective case series study at a tertiary referral center.
Methods
Twenty-four Trab-MMCs and twenty-three GATTs performed in patients with primary open-angle glaucoma (POAG) between January 2019 and January 2022, with regular follow-up for at least one year, were retrospectively analysed. Intraocular pressure (IOP) , number of antiglaucomatous drugs, intaoperative and postoperative complications and success rates were recorded for both surgeries.
Results
In the Trab-MMC group, a statistically significant decrease was achieved in the final mean IOP (12.17 ± 2.99 mmHg) compared to the mean IOP in the preoperative period (26.29 ± 1.57 mmHg) (P=0.001). In the GATT group, a statistically significant decrease was achieved in the final mean IOP (13.48 ± 0.07 mmHg) compared to the mean IOP in the preoperative period (25.91±3.69 mmHg) (P=0.001). There was no statistically significant difference in IOP reduction between the two groups (p<0.087).In the Trab-MMC group, a statistically significant decrease was achieved in the final mean antiglaucomatous medications number (0.83 ± 0.73) compared to the mean antiglaucomatous medications number in the preoperative period (3.79±0.50) (P=0.001). In the GATT group, a statistically significant decrease was achieved in the final mean antiglaucomatous medications number (1.47 ± 1.05 ) compared to the mean antiglaucomatous medications number in the preoperative period (3.73±0.68) (P=0.001).There was no statistically significant difference between Trab-MMC and GATT in reducing the number of antiglaucomatous medications taken (P<0.087).The predominant complication observed in the Trab-MMC group was filtration failure, while the GATT group showed a higher incidence of hyphema. However, there was no significant difference in the rate of complications between the two groups (P<0.092)
Conclusion
Although GATT is a minimally invasive surgery, it can be said to be a safe and effective alternative to Trab-MMC in POAG due to similar IOP reduction and reduction in the number of antiglaucomatous in the 1st year.
