Purpose
To assess rotational stability, refractive outcomes, decentration and tilt of a new toric hydrophobic acrylic intraocular lens (IOL)
Setting
This single-center, prospective clinical trial was conducted at the Department of Ophthalmology, Medical University of Vienna.
Methods
Cataract patients with a total corneal astigmatism of more than one diopter measured by AS-OCT (MS39,CSO, Firenze, Italy) were included. Patients received a hydrophobic acrylic toric Clareon CN0T3-9 (Alcon, Forth Worth, TX) IOL either in one eye or in both eyes. Baseline measurement was performed at the end of surgery (EOS) with the patient still supine on the operating table using non-movable landmarks. Postoperative retroillumination pictures were taken at 1 hour (1h), 1 week (1w), 1 month (1m) and 4- 6 months (6m). Decentration and tilt were measured with an AS-OCT Casia 2 (Tomey, Nagoya, Japan) at preop, 1w and 6m. Subjective manifest refraction was assessed at the 6m follow up visit. Data is presented as mean± SD [min;max].
Results
Final results were obtained in 126 eyes of 80 patiens. Mean absolute rotation from EOS to 6m was 1.33± 2.0 [0.01;19.80] degrees. Rotational stability values from EOS to 1h, 1h to 1w, 1w to 1m and 1m to 6m were 0.86±0.82 [0.0;3.90], 1.06±1.94 [0.0;19.45], 0.47±0.42 [0.0;2.03] and 0.38±0.40 [0.0;2.56] degrees. Decentration and tilt of the crystalline lens were 0.17±0.1 mm and 5.7±1.6 degrees, respectively. Decentration and tilt of the pseudophakic lens were 0.24±0.13 mm and 5.6±1.5 degrees at 1w and 0.25±0.15 mm and 5.8±1.5 degrees at 6m, respectively. Mean absolute preoperative corneal astigmatism was +1.78± 0.83D. Mean absolute postoperative refractive astigmatism was +0.33±0.27D at 6m.
Conclusion
The Clareon toric IOL presented very good rotational stability with a mean absolute rotation below 1.4° from end of surgery to 6 months. Only two IOLs rotated more than 5° with no repositioning surgery necessary. Decentration and tilt were generally low compared to preoperative crystalline lens decentration and tilt values. Refractive outcomes were satisfying with a mean absolute residual refractive astigmatism clearly below 0.5 diopters.
