Purpose

Postoperative spectacle independence is becoming increasingly imported for patients undergoing cataract surgery. Implantation of extended depth of focus (EDOF) IOLs enhances uncorrected intermediate visual acuity with less photopic phenomena. The present study compared clinical performance an EDOF IOL (Acunex Vario AN6V) with the monofocal version of this IOL (Acunex AN6).

Setting

Vienna Institute for Ocular Surgery (VIROS), a Karl-Landsteiner institute, Hanusch Hospital, Vienna, Austria

Methods

Fifty patients (100 eyes) were included into this prospective study. The patients were randomized to receive either the Acunex Vario AN6V (EDOF-Group) or the Acunex AN6 (Monofocal-Group), bilaterally. The target refraction of all eyes were set to emmetropia (-0.25D) using the Haigis formula. The baseline examination was performed prior to the surgery and three follow-up visits were scheduled (one week, two months and six months after surgery). Uncorrected as well as distance corrected far, intermediate and near visual acuity were determined. In addition, stereovision assessments, defocus curves and questionnaires on spectacle independence and satisfaction were done.

Results

Distance visual acuity was comparable between both groups. Intermediate and near visual acuity was better in the EDOF group both without and with distance correction. Stereovision improved significantly in both groups after cataract surgery but was not different between groups. The most common complaint regarding quality of vision was glare in both groups. Halos, on the other hand, were slightly more prevalent in the EDOF group; however, complaints were only mild to moderate. Fewer patients in the EDOF group wore glasses all the time and more patients saw well at close range without glasses.

Conclusion

Spectacle independence is increased using the ACUNEX Vario (AN6V) in comparison to a monofocal IOL (Acunex AN6). Thereby, intermediate and near vision improved. Stereovision and quality of vision was comparable between both groups. Nevertheless, halos were slightly more frequent in the EDOF-Group – but complaints were only mild to moderate.
