Purpose
This is the first clinical experience of trifocal IOL (Triva aXAY, HumanOptics) with a 7 mm large optic with a low number of diffractive rings with near additions +1.75 D and +3.5 D. Optic is designed to minimize dysphotopsia even in larger pupils. The IOL is made of hydrophilic acrylic.
Setting
Gemini Eye Clinic, Zlin, Czech Republic
Methods
32 eyes (16 patients) with a median age of 58 years (IQR,54;64) were implanted with Triva aXAY (HumanOptics) IOLs as part of routine cataract surgery. Follow-up was set at 6 months and included spherical equivalent refraction (SEq.), Uncorrected and Corrected Distance Visual Acuity (UDVA, CDVA), Uncorrected and Distance Corrected Intermediate Visual Acuity (UIVA, DCIVA) at 66 cm, Uncorrected and Distance Corrected Near Visual Acuity (UNVA, DCNVA) at 40 cm. All were assessed as monocular and binocular. Defocus curves with vergence at -4.0 D and +1.0 D. Perceived halo/glare were evaluated using a questionnaire. Values are expressed as median with IQR (25;75%).
Results
At 6 months mean (SD) SEq. was 0.18 ± 0.47 D. Monocular CDVA were 0.0 logMAR (0.0; 0.02), DCIVA 0.11 logMAR (0.04; 0.20) and DCNVA 0.05 logMAR (0.00; 0.12). Binocular DCIVA was 0.02 logMAR (-0.06; 0.08) and DCNVA of 0.00 logMAR (-0.04; 0.04). 94 % of patients read at least 0.2 logMAR or better at binocular DCIVA and all patients read at least 0.2 logMAR at binocular DCNVA. Defocus curve showed that binocular visual acuities of 0.2 logMAR or better were on average maintained between -3.5 D and +0.75 D. In total 36 % of patients have no experience of photic phenomena while only 7 % of patients perceive strong halos, 21 % strong glare and 29 % strong starburst.
Conclusion
Initial clinical results have demonstrated excellent performance in terms of visual acuity and defocus curve, encompassing both near and intermediate vision. Notably, over one-third of patients reported no dysphotopsia while low numbers of patients reported strong halo & glare effects. This lens may consequently offer the anticipated level of vision quality sought by individuals with high expectations and a desire for freedom from spectacles.
