Purpose
To compare the effects of corneal allograft intrastromal ring segment implantation (CAIRS) on tomographical measurements as well as visual and refractive outcomes of keratoconic eyes with and without prior corneal crosslinking (CXL).
Setting
Retrospective study
Methods
Retrospective study of 67 eyes with sterilized and decellularized CAIRS (KeraNaturalTM, VisionGift, Portland, OR, USA) implantation due to advanced keratoconus were included. Of these, 37 eyes had no CXL and 30 eyes had CXL before being referred to us. Changes in spherical equivalent (SE), uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), K1, K2, Kmean, Kmax, and thinnest pachymetry 6 months after the implantation were evaluated.
Results
All tomographical, visual acuity and refractive parameters before implantation were similar in both groups (p>0.05 for all). At 6 months, CDVA, K1, and Kmean showed higher improvement in non-CXL group than CXL-group (p=0.030; 0.018; 0.039 respectively).
Conclusion
CAIRS surgery has a flattening effect on the cornea. Crosslinked corneas had less flattening effect, arguably due to the stiffing effect of prior CXL. Therefore, to achieve maximum flattening effect and improvement in CDVA, CAIRS surgery should be applied before CXL.
