Purpose
To compare the reliability and validity of ocular-surface disease index questionnaire (OSDI) and non-invasive objective assessment of the tear film uusing the Oculus® Keratograph 5M to identify patients with abnormal tear film before premium cataract surgery.
Setting
prospective observational descriptive clinical trial
Methods
We carried out a prospective observational descriptive clinical trial in patient candidates for premium cataract surgery with implant of multifocal intraocular lens (MIOL). The inclusion criteria were as follows: clinical diagnosis of senile cataract proposed for premium cataract surgery with MIOL, with and without dry-eye symptomatology. To identify dry-eye symptomatic or asymptomatic patients the ocular-surface disease questionnaire (OSDI) was filled out during the preoperative visit. The non-invasive objective assessment of the tear film was carried out with corneal topograph Oculus® Keratograph 5M meassuring noninvasive keratograph breakup time (NIKBUT), recording the time for the first breakup (NIKBUT f) and the breakup time affecting at least 4 adjacent quadrants in the cornea (NIKBUT av). The results of OSDI and NIKBUT were analyzed and compared in asymptomatic and symptomatic group.
Results
150 patients were included in the study, 63 without dry-eye symptomatology and 87 with symptoms. In asymptomatic group (control) the mean age was 61,30 ± 8,20 years old (r 57 – 74) and in the symptomatic group 69,35 ± 9,80 years old (r 61 – 76). In the control group the NIKBUT (f) showed breakup time at 10.09 ± 6.54 sec and NIKBUT (av) at 13,04 ± 7.33 sec. The symptomatic group showed NIKBUT (f) values of 4.97 ± 4.22 sec and NIKBUT (av) of 8,02 ± 6.74 sec. The assessment of tear film stability according to the different levels was as follows: in the symptomatic group 25,33% presented level 0, 16,0 % level 1, 37,33 % level 2 and 21,33 % very short breakup time. The values recorded in asymptomatic group (control) were level 0 42,0%, level 1 33,33%, level 2 16,0%, very short breakup time 8,66%. The results in the symptomatic group are consistent whereas in the control group 24,66 of asymptomatic patients had abnormal tear film stability.
Conclusion
Regarding the results of this study the ocular-surface disease index questionnaire (OSDI) and tear film NIKBUT values were weakly correlated. Knowing only the OSDI questionnaire scores, 1 in 4 asymptomatic patients would be included for premium cataract surgery with abnormal tear film. Non-invasive objective assessment of the tear film is needed to confirm the tear film stabilty.
