Purpose
To optimize the results of integration of a new excimer laser in a daily practice of a refractive surgeon.
Setting
Sphere clinic, Moscow, Russia,
Methods
A prospective study of two groups: 1 group -patients who underwent LVC with SCHWIND Amaris excimer laser with a 500 Hz repetition rate (TransPRK 437 eyes and FemtoLASIK - 173 eyes), 2 group - LVC with SCHWIND Amaris with 1050 Hz repetition rate (170 and 43 eyes accordingly). Flap was performed using a VISUMAX femtosecond laser. The surgery was planned with a SCHWIND CAM software. The follow-up observation control points: day of epithelialization 3-4 days (for TransPRK) or 1 week, 1,3 and 6 months after surgery). Statistical analysis was performed with 95% confidence interval using the Mann-Whitney test.
Results
The following statistically significant differences were found: UDVA 1 month postop. for FemtoLasik patients and 3 months postop. for TransPRK patients was higher in 2-d group, the predictability was higher in 2-d group, the effectiveness of astigmatism correction for FemtoLASIK patients was higher in 2-d group with a slight overcorrection for patients with cylinder more than 3 D. We observed some transitory hazes with a degree not more than 0,5 in 3,2% in first group and 2,4% in the 2-d group. We didn’t find any statistically significant difference in safety and accuracy in both groups for all types of surgery.
Conclusion
Excellent results in refractive surgery requiring constant updating of nomograms. Changing the laser model, any changes in operating room conditions, disinfection, optics opacification, changes in surgery technic - all these can lead to unexpected refractive result which should be detected as soon as possible. To anticipate the deviation of the obtained result from the expected one, it is necessary to have a thorough understanding of the technical principles of the excimer laser and in particular the specific model used as well as the available options and features of the device and how they can influence the clinical result.
