Risk Factors For The Occurrence Of Intraoperative Floppy Iris Syndrome

Purpose
The purpose of this research was to asses risk factors for intraoperative floppy iris syndrome (IFIS) in patients undergoing cataract surgery.
Setting
This study was conducted at University Department of Ophthalmology, University Hospital Center "Sestre milosrdnice“
Zagreb, Croatia, in the period from April 2022 to December 2022.
Methods
A total of 488 eyes were enrolled in the study, and all patients underwent uncomplicated cataract surgery. Intraoperatively, patients were divided into two groups, IFIS group and non-IFIS group, based on the criteria described by Chang and Campbell. According to the severity of their IFIS, patients were divided into three groups. The following data was analyzed: gender, age, ophthalmic conditions, systemic diseases, systemic therapy and duration of use, and preoperative pupil size.
Results
IFIS was identified in 12% of patients, of whom 21.6% had an IFIS grade 1, 51.6% had an IFIS grade 2, and 26.6% had a full IFIS. Risk factors for the development of IFIS include male gender, old age, use of adrenergic receptor antagonists, use of hypertension drugs, and preoperative narrowing of the pupil. Men exposed to a 1a antagonists had a statistically significantly higher risk of developing IFIS and a more severe form of IFIS. Preoperative pupil size correlates with IFIS severity.
Conclusions
Understanding and identifying factors that contribute to the risk of IFIS development is critical for identifying potential IFIS patients.
