Purpose

To evaluate postoperative visual outcome for enhanced monofocal IOL in comparison to monofocal IOL after bilateral cataract surgery.

Setting

Specialized ophthalmic center, Baghdad, Iraq

Methods

A comparative study with 40 patients after uncomplicated cataract surgery with <1.5 D of corneal astigmatism, 20 patients with bilateral implantation of TECNIS Eyhance ICB00 IOL, compared to 20 patients with bilateral implantation of AcrySof SA60AT IOL. Measurement of uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), uncorrected 
intermediate visual acuity (UIVA), distance corrected intermediate visual acuity (DCIVA) at 70 cm, uncorrected near visual acuity (UNVA) and distance corrected near visual acuity (DCNVA) at 40 cm, binocular defocus curve, binocular contrast sensitivity, wavefront aberrometry, adverse effects and subjective visual quality using the validated visual function questionnaire VF-14.

Results

There was no significant difference in the postoperative spherical equivalent (SE) between the two groups while Eyhance group showed significantly better monocular and binocular UIVA, DCIVA and UNVA with comparable monocular and binocular UDVA and CDVA. Patients with Eyhance achieved better visual acuities (VA) throughout negative defocus range (from – 0.5 to – 4.0 diopters) in the binocular defocus curve. Angle alpha found to be larger in Eyhance group while Angle kappa and total spherical aberration found to be larger in Alcon group.Contrast sensitivity and photic phenomena outcomes were similar, regarding VF-14 questionnaire, patients with Eyhance had significant less difficulty in reading books,seeing steeps/stairs, and watching TV. 

Conclusions

Enhanced monofocal IOL is as effective as monofocal IOL for distant vision, with improved function for intermediate and near distances, comparable photic phenomena prevalence and better performance in daily life.
