Purpose

To evaluate visual outcomes and patient satisfaction after bilateral cataract surgery with implantation of an enhanced monofocal intraocular lens.

Setting

Ophthalmology Department. Hospital Clínico San Carlos. Madrid. Spain.

Methods

Observational, prospective study on 50 eyes of 25 patients who had an enhanced monofocal intraocular lens (RayOne EMV, Rayner Intraocular Lenses Limited, Worthing, UK) implanted bilaterally. The lens provides a positive spherical aberration to increase depth of focus.Visual results were evaluated 1 and 3 months postoperatively and included binocular uncorrected distance acuity (UDVA) at 4 m, corrected distance acuity (CDVA) at 4 m, uncorrected intermediate visual acuity (UIVA) at 66 cm, and corrected intermediate visual acuity (CIVA) at 66 cm. A binocular defocus curve (+1.00 D to -2.50 D, in 0.50 steps) and questionnaires to assess patient satisfaction (Catquest-9SF; McAllinden, QoV) were obtained. We used the Barrett Universal II Formula.

Results

The binocular UDVA, CDVA and UIVA under photopic conditions were 0.14 ± 0.20, -0.05 ± 0.07 and 0.19 ± 0.11 LogMAR respectively at the 1-month follow-up. The values obtained for the same visual acuities were 0.03 ± 0.10, -0.06 ± 0.07 and 0.14 ± 0.11 respectively 3 months after surgery.  The binocular defocus curve in photopic conditions showed values better than 0.3 between +1.00 D and -1.50 D. 96% of the patients did not refer haloes or experienced them only occasionally, and 98% reported a high degree of satisfaction.

Conclusions

The implantation of enhanced monofocal intraocular lens bilaterally, RayOne EMV, showed an excellent far and intermediate vision with high patient satisfaction results.
