Longterm Outcomes Following Cionni Ring Bag Fixation And In-The-Bag Iol Implantation: Prospective Study

Purpose

PURPOSE:

To report the longterm outcomes following Cionni modified capsule tension ring (CTR) fixation of the capsular bag and in-the-bag IOL implantation

Raghudeep eye Hospital, Ahmedabad, India

Methods

METHODS:

Prospective, observational case series. The study population comprised 30 eyes with subluxated lenses that underwent lens extraction, capsular bag fixation with Cionni CTR, and in-the-bag IOL implantation. All eyes completed minimum 10 years follow-up. Main outcome measures were long term postoperative best-corrected visual acuity (BCVA), IOL centration, and complications

Results

RESULTS:

7 year follow up results were: mean age-29.4 years. Marfan’s Syndrome was the most common cause of subluxation (65%).Anterior vitrectomy performed in 2 eyes.22eyes had a Single element and 8 eyes Double element Cionni modified capsule tension ring for bag fixation.Mean follow-up:90.1 ± 4.9 months.Mean preop BCVA:0.66±0.22 logMAR improved to 0.33 ± 0.21 logMAR at final visit (P<.001).BCVA improved in 85% eyes.IOL decentration noted in 4 eyes(13.3%),requiring repositioning in 3 eyes.30% eyes required Nd:YAG capsulotomy.Other complications included cystoid macular edema in 1(3.3%),posterior synechiae in 2 (6.6%),retinal detachment in 2(6.6%) eyes. 10 year followup results will be updated.

Conclusions

CONCLUSION:

In-the-bag implantation of a Cionni CTR with IOL appears to be a safe option in eyes with subluxated cataract, ensuring a stable IOL with acceptable rate of complications in the long term.
