Purpose

To evaluate 3-months postoperative results and patient satisfaction after presbyopic refractive lens exchange (RLE) with blended monovision implanted with non-diffractive, extended depth of focus (EDOF) intraocular lenses (IOL).

Setting

Multicenter, multiple-surgeon, private practice, Scandinavia (Denmark, Norway and Sweden); Memira Eyecenter.

Methods

Prospective analysis of 75 patients (150 eyes) undergoing presbyopic RLE with a controlled positive spherical aberration IOL (Rayner RayOne EMV). We included patients that were not recommended for trifocal IOLs due to lifestyle or work situation, small anatomical anomalies, or due to unwillingness to compromise optical quality due to diffractive optics. Refractive target was modified monovision (emmetropia in the dominant eye and -1.50D in the reading eye), with the increased range of focus creating a blended vision for intermediate distance. Refraction, uncorrected visual acuities at distance (UDVA), intermediate (UIVA), near (UNVA), and patient reported outcome measure (PROMs) questionnaire were collected at 3-months postoperatively.

Results

Twenty-eight of 75 patients have been collected to date. The mean binocular UDVA, UIVA and UNVA were 0.02 ± 0.11, 0.33 ± 0.09, 0.16 ± 0.11 LogMAR, respectively. Distance target eyes UDVA: 0.06 ± 0.15, UIVA: 0.48 ± 0.04 and UNVA: 0.49 ± 0.14 LogMAR. Near target eyes UDVA: 0.53 ± 0.2, UIVA: 0.35 ± 0.15 and UNVA: 0.17 ± 0.09 LogMAR. 

Conclusions

The optical design of the RayOne EMV IOLs can provide an enhanced range of focus and serve as an optical solution for presbyopic RLE patients. The optic design provides a lens alternative with high patient satisfaction and spectacle freedom also for patients unsuitable for trifocal IOLs.
