Purpose

To compare the visual outcomes and the optical quality in patients with bilateral implantation of extended depth of focus (AcrySof® IQ Vivity DFT00) IOL and premium monofocal (Tecnis® Eyhance ICB00) IOL.

Setting

Department of Ophthalmology, Samsung Medical Center, Sungkyunkwan University School of Medicine, Seoul, Republic of Korea

Methods

We retrospectively analyzed 170 eyes of 85 patients who underwent bilateral cataract surgery with AcrySof® IQ Vivity IOL or Tecnis® Eyhance IOL at Samsung Medical Center between May 2021 and November 2022. Preoperative and postoperative manifest refraction, monocular uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA) were recorded. Uncorrected intermediate visual acuity (UIVA) at 60cm and 80cm, and uncorrected near visual acuity (UNVA) at 33cm and 40cm were also analyzed at 1, 3, 6 months postoperatively. Binocular defocus curve, contrast sensitivity, spectacle independence, satisfaction, and visual symptoms were analyzed at 3 months postoperatively.

Results

43 patients and 42 patients were included in Vivity group and Eyhance group, respectively. The mean monocular postoperative visual acuities at all distances and refractive outcomes were comparable between each group (p>0.05). Compared to Eyhance group, Vivity group provided the broader depth of focus. There were no differences in contrast sensitivity before and after surgery in both groups. Spectacle independence and satisfaction at near distance were higher in Vivity group than in Eyhance group. No significant differences were detected between groups in visual symptoms

Conclusions

Bilateral implantation of Vivity IOL and Eyhance IOL were similarly effective in visual acuity with minimal visual disturbances at all distances. However, Vivity IOL showed the more increased depth of focus and higher spectacle independence at near distance than those of Eyhance IOL.
