Opposite Clear Corneal Incisions Vs Steep Axis Incision In Reduction Of Total Corneal Astigmatism Undergoing Phacoemulsification

Purpose

To compare reduction in total corneal astigmatism (ΔTK) after opposite clear corneal incisions (OCCI) on steep meridian (Group 1), and conventional single main port incision on steep meridian (Group 2) in patients undergoing phacoemulsification with monofocal non-toric PC-IOL implantation.

Setting

Retrospective study

Methods

34 cataractous eye underwent phaco with PCIOL. In both the group, 17 eyes included. in first group incision with 2.2 mm keratome given on steep axis while in other group Opposite clear corneal incision (OCCI) along with 2.2 mm steep axis incision was given. Total keratometry on IOL master 700 calculated between the two group at 1week, 1month and 3 month.

Results

Mean pre-op ΔTK was 1.48±0.48D in Group 1 and 0.77±1.4D in Group 2. Post-op ΔTK at week 1 was 1.02±0.47D and 0.44±0.15D, and at one month 0.91±0.49D and 0.54±0.14D in the two groups respectively (p<.00001). correction of 0.57±0.44d was found in group 1 and 0.23±0.02d 2 (p="0.00169).

Conclusions

OCCI on steep axis can be helpful in correcting lower degree of astigmatism without use of any additional instrument and extra cost.

It improves quality of life by better uncorrected visual acuity
