Purpose

To compare the ease of flap lift and visual outcomes after laser-in situ keratomileusis (LASIK) performed with the ELITA femtosecond laser system (ELITA) (Johnson & Johnson Surgical Vision, Irvine, CA) versus Intralase iFS for compound myopic astigmatism.

Setting

This clinical trial is part of a new FS laser development project (ELITA) and is performed at 2 clinical sites in India. This is a randomized, single arm, contra-lateral, prospective treatment study involving subjects 18 years or older with refractive errors within LASIK treatment indications.

Methods

In this multicentre, prospective, randomised clinical trial which was conducted at three sites, 83 consecutive subjects received LASIK surgery using the ELITA laser system in 1 eye and iFS in the fellow eye for flap creation. Ease of flap lift for each subject was evaluated by the surgeons using the following grading scale: 1 = iFS is much better than ELITA, 2 = iFS is superior to ELITA, 3 = ELTIA is similar to iFS, 4 = ELITA is superior to iFS, 5 = ELITA is much better than iFS. Uncorrected visual acuity (UCVA), manifest refractive spherical equivalent (MRSE), and best spectacle-corrected visual acuity (BSCVA) were obtained at 1 month postoperatively.

Results

The ease of flap lift was graded 3 or higher in 98% of the cases, with 71% of them graded as 4 or 5 during the procedure (n=83). At 1-month, 97% of ELITA eyes and 94% of iFS eyes were within +/- 0.50D of target refraction. UCVA was 20/20 or better in 94% of ELITA eyes and 88% of iFS eyes at 1 day (n=83), and in 98% of ELITA eyes, and 95% of iFS eyes at 1-month (n=64). BSCVA was 20/20 or better in 100% of both ELITA and iFS eyes at 1-month post op. 

Conclusions

The ELITA femtosecond laser system demonstrates equivalent or easier flap lifting compared to iFS when creating flaps as well as excellent visual outcomes for LASIK.
