Both LASIK and Smile are safe and effective inmyopic refractive corrections; This studycompares the long-term safety and efficacy of topography-guided LASIK vs. contralateral eye SMILE formyopia and myopic astigmatism correction. This contralateral eye study included 44 eyes of 22 patients 22 eyes were treated withtopography-guided LASIKand the fellow eye of each patient was treated with SMILE. The followingparameters were evaluated preoperatively and up to 48 months postoperatively: uncorrected distancevision acuity (UDVA), corrected distance vision acuity (CDVA), refractive error, corneal keratometry,contrast sensitivity,and retreatments
At 48 months, 92.4% of the topography-guided LASIKgroup and 79.2% of the SMILE grouphad UDVA of 20/20 (P < .002) and 62.3% and 34.5%, respectively, had UDVA of 20/16 (P < .002).Spherical equivalent refraction (±0.50 D) was 95.5% for the LASIK group and 76.3% for the SMILEgroup (P < .002). Residual refraction cylinder (≤ 0.25 D) was 81.8% for the LASIK group and 50% for theSMILE group (P < .001). Two eyes of theSMILE group underwent PRK retreatment.
LASIK and SMILE are both lamellar femtosecond laser assisted laser refractive cornealprocedures. Long term comparison of the two techniques in a randomized contralateral eye study bothdemonstrated goodefficacy and safety. LASIK topographic customization appears to offer superioraccuracy.Topography-guided LASIK was superior in all visual performance parameters studied, overlong-term follow-up
