Comparison Of Clinical Efficacy Of Phakic Implantable Collamer Lens (Icl) Implantation With And Without Use Of An Ophthalmic Visco Surgical Device (Ovd)

Purpose
To compare the clinical efficacy of posterior chamber phakic implantable collamer lens (ICL) implantation with and without the use of an ophthalmic viscosurgical device (OVD).
Setting
Cure Sight LAser Centre, Ahmedabad, India
Methods
Patients who underwent ICL implantation with the use of an OVD (OVD group) and without the use of an OVD (hydro group) were included. Refractive error, endothelial cell density (ECD), intraocular pressure (IOP), vault height, and anterior chamber reaction were evaluated at day 1, day 15, 3 months, and 6 months postoperatively. Primary outcomes as UDVA, CDVA were noted. The IOP and vault height were checked 1 hour postoperatively and the surgical time was recorded.
Results
The OVD group comprised 50 eyes of 27 patients and the hydro group comprised 50 eyes of 25 patients. There were no significant preoperative differences between the 2 groups. At 1 hour postoperatively, the mean IOP in OVD group and hydro group was 24.6 ± 3.09 and 18.1 ± 2.64 mmhg respectively (P=0.00). However, there was no significant difference in IOP between the 2 groups at other visits. Surgical time was significantly shorter for the hydro group (P = 0.00). There was no statistically significant difference in post operative vault, ECD, or refractive error between the 2 groups during the 6 month follow-up. 1 eye underwent ICL rotation at 10 days in OVD group and 2 eyes had high vault (>750 micron). No adverse events noted in hydro group.
Conclusions
ICL implantation without the use of an OVD was safe and effective. The advantages being the short surgical time and prevention of early postoperative IOP spikes. Rest parameters were comparable between two groups at all followup visits. It can also be cost effective if one omits OVD during surgery.
