Ptrygium Removal Ended In Enucleation, The Worst Senario!

Purpose

 
Infectious endophthalmitis a tragedic event, is most commonly
exogenous, in which the organism enters the globe through a defect
in eye ball, happened by trauma or surgery. While exogenous
endophthalmitis happens most commonly after cataract surgery, but
it could do happen after many other ocular surgeries like vitrectomy,
sclera buckling or even strabismus surgery. In pterygium surgery just
the globe surface would be traumarized, so happening of
endophthalmhitis has been rarely reported. Herein we would
report a case of aspergillus fumigatus endophthalmitis after
pterygium removal with Mitomycin application and inadvertently globe dissection during the surgery, which finally ended in enucleation.

Setting

The file recordings of the patient had been reviewed and surgeries was performed at Emam hospital, kermanshah university of medical sciences, kermanshah, Iran.

Methods

It is a Retrospective interventional case report. 

A 55 year-old man referred to the eye clinic complaining of the left
eye pain and decrease vision since 3 days ago. The past medical
history was unremarkable, but the past ocular history revealed a mitomycin C- applicated free flap pterygium removal about 3 week ago. Pars plans deep vitrectomy and intravitreal antibiotic injection performed for the patient but due to severe panophthalmitis the patient got enucleated finally

Results

Although Ptrygium removal is generally a safe surgery but devasting complications like endophthalmitis could rarely happen, 

Conclusions

regular follow up and timely management of complications are mandatory in each ocular surgery like Ptrygium removal
