Mucus Membrane Grafting For The Novice Surgeon

Purpose
To discuss the hurdles experienced by beginners in Mucus Membrane Grafting (MMG) for Lid Margin Keratinization (LMK)

Methods
2 patients post Stevens-Johnson Syndrome (SJS) aged 58 and 31 years presented with pain, foreign body sensation, and poor eye-opening bilaterally. Differential fluoresceine staining was seen in both patients suggesting lid margin keratinization-induced keratopathy. Bilateral MMG was performed in both the upper and lower lids in both patients.
Results
We experienced excessive bleeding of the recipient bed, difficult dissection of the tarsal conjunctiva, a small central corneal epithelial defect and button-holing of the graft which were managed intraoperatively. Postoperatively, both patients were given topical antibiotics and lubricants. Both patients had symptomatic improvement in their ocular symptoms and better eye-opening.
Conclusions
Mucus membrane grafting can be a ‘Game Changer’ by modifying the course of chronic SJS sequelae cases with LMK-associated keratopathy. It is essential for a beginner to understand the steps of MMG to ensure a good clinical outcome and prevent snowballing toward keratoprosthesis in these eyes.
