Purpose

Some cases of keratoconus, especially asymmetric ones may not do as well with uniform thickness intracorneal ring segments. Customized CAIRS (Corneal Allogenic Intrastromal ring Segments) aims at being able to tailor CAIRS to each individual patient’s topography so as to get the best possible individualized outcome tailormade for each patient.

Setting

Dr. Agarwal's Eye Hospital, Chennai, India

Methods

CAIRS customization was done based on the patient's topography. More volume was added where greater flattening was required. Various different shapes were used depending on topographic pattern.

Results

Customized CAIRS gave significant imprtovements in uncorrected and spectacle corrected distance visual acuity and decraesed comatose aberrations. An association between greater volume implantation resulting in greater flattening was seen.

Conclusions

Customized CAIRS tailors CAIRS to individual patient’s topographical and refractive requirement. Customization can include among others, specifically shaped CAIRS, symmetric or asymmetric CAIRS, progressive or variable thickness CAIRS, progressive and/or variable width CAIRS, combinations, sudden or gradually tapered shape transitions, variations in extent and location of shape change along the arc length etc.
