To evaluate CXL-line depth in keratoconic eyes following surface ablation normalization combined with 4 different corneal crosslinking fluence protocols(Athens protocol)
LaserVision Ambulatory Eye Surgery Unit, Athens, Greece
All cases were documented progressive keratoconus and treated with Athens Protocol CL: excimer laser suface remodeling combined with 4 different UV fluence protocols for CXL: 6mW/cm2 X 15 minutes-group A, 10mW/cm2 X 10 min-group B, 18 mW/cm2 X 6 min-group C and last 30mW/cm2 X 4 min-group D
Fourier-domain anterior segment optical coherence tomography (AS-OCT) was used to obtain in vivo 3-dimensional corneal and epithelial thickness maps and as well as cross-section assessment of the average depth and width of the CXL derived intrastromal-line visualized
95 treated keratoconic eyes were evaluated over 3 years CXL line mean depth: The number of cases and mean depth of CXL-line documented by OCT at 1 month was for each group: A: 28 cases and 265um (+/- 35), B: 20 cases and 205um (+/- 25 um), C: 21 cases and 140um (+/- 20um) and last D: 26 cases and 110um (+/- 20 um)
These data confirm our previous findings of documentation of CXL-line depth , and an objective marker of the effective CXL achieved appears to be respective of the UV fluence utilized in the CXL part of the Athens Protocol: lower fluence for longer time exposure results in deeper effect. The clinical significance of these findings remains to be investigated.
