Purpose

To evaluate the effectiveness of the Predescematic corneal sutures combined with  intrastromal puncture evacuation of edema and  injection of gas in the anterior chamber to fasten the resolution of corneal edema as primary  treatment of acute hydrops

Setting

Memorial institute for ophthalmic research

Methods

A retrospective study was done on 4 cases of sever acute hydrops with sever corneal edema involving more than 8ml of the cornea were  vision dropped to  HM . Intrastromal puncture evacuation of edema and anterior chamber injection of gas were done in two cases  as primary treatment while the other 2 cases this was combined with Predescematic sutures . Cases were  evaluated for rate of edema resolution and visual improvement .Follow up was done with anterior segment OCT   

Results

Corneal edema decreases in all cases on  early days postoperative with more reduction in the cases were Predescematic sutures were taken . The rate of reduction of corneal edema was faster with the sutures . pain improved in all cases early postoperative . visual improvement ranges from CF 60cm to 2/60

Conclusions

primary surgical treatments for sever acute hydrops  fasten the rate of recovery and decrease  complications .Predescematic sutures combined with  intrastromal puncture  evacuation of edema and  injection of gas in the anterior chamber give better results and faster recovery than intrastromal punctures and gas injection alone
