Purpose

To describe the surgical maneuvers to unscroll young donor corneas and the utility of an intraoperative optical coherence tomography (OCT in Descemet Membrane Endothelial Keratoplasty (DMEK) surgery.

Setting

Hospital Clinico San Carlos, Madrid, Spain

Methods

Six DMEK surgeries were performed in a tertiary center. Corneal donor ages: one was 21, two were 22, two were 26 and last one was 30 years old. Two patients had bullous keratopathy, one of which was a failed DSO (Descemet Stripping Only) and 4 patients had pseudophaquic Fuchs dystrophy. Video images and real time OCT images were analyzed to explain different steps, tips and tricks of the surgery including graft preparation and unscrolling such young donors. Surgical time and unfolding time were analyzed during surgery. Visual acuity and endothelial cell count were analyzed in the postoperative period.

Results

All six patients underwent uneventful DMEK surgeries with a greater improving in visual acuity. Grafts were succesfully unscrolled, orientated and put in place by using intraoperative OCT. No rebubbling was needed in any of the cases. We found no differences in surgical time and unfolding time with other DMEKs surgeries performed with older donors.

Conclusions

DMEK rolls from younger donors are much tighter but with experience and technology surgery can be well performed by a trained surgeon.
