Our Experience And Management Of Patients With Infectious Crystalline Keratopathy (Ick )At Malayan Eye Clinic.

Purpose
The aim of this paper is to share our experience and report our approaches to treatment.
Setting
In the period of 2013-2022, 37 patients with ICK were admitted to the Сornea-Uveitis department at Malayan Eye center, Yerevan, Armenia
Methods
A retrospective review of the medical records of patients with Infectious Crystalline Keratopathy (ICK) were conducted between January 2013 and December 2022. Most of the cases were observed after penetrating keratoplasty, caused by Streptococcus viridians group. There was one case after pterygium excision caused by Mycobacterium chelonae and one case with history of penetrating trauma, PKP, secondary IOL implantation caused by Corynebacterium spp. The earliest case with ICK was observed 3 months after penetrating keratoplasty, also we had Candida caused case on corneal transplant that amazingly responded to antifungal drops just in 3 weeks.
Results
According to our practice, out of 120-150 annual corneal transplant surgeries, 4 cases of ICK were observed annually. Due to long-term medical treatment, full recovery was observed in all patients․
Conclusions
Based on the results of the treated cases, we came to conclusion that ICK can occur as early as, 3 months after the surgical intervention and medical treatment can lead to satisfying results, even if it takes long-term. Based on our experience, all cases responded well to moxifloxacin, despite long-term treatment (9 months or more), we still managed to avoid repent graft surgery and observed complete healing of all corneas.Only one case underwent also amniotic membrane transplantation. Patients were left with mild scarring not causing much visual impairment.
