To assess the surgical indication of a challenging case of infectious fusarium keratitis.
We report a case of an adult male wearing contact lenses who developed a torpid episode of infectious keratitis, with inflammatory involvement in the anterior chamber.
A microbiological and pathological diagnosis of fusarium keratitis was made. Due to the lack of control of the infectious process with medical treatment a penetrating keratoplasty was performed to avoid further extension to the sclera
The surgical procedure was performed without complications and the postoperative course was satisfactory, best corrected visual acuity (BCVA) 0.5 decimal
Fusarium infection is usually more common in tropical climates, early diagnosis and treatment are the key to improving response and avoiding the spread of infection and severe complications such as endophthalmitis; antifungal resistance is very common in the Fusarium genera. Hence, surgical treatment can help to obtain a successful result.
