Results From A Contralateral Eye Study Using The Hoya Gemetric And Gemetric Plus Trifocal Intra-Ocular Lenses

Purpose
To determine the visual outcomes and range of vision using the Gemetric IOL from Hoya in the dominant eye, and the Gemetric plus IOL in the non-dominant eye.
These are trifocal IOL's with different light distribution curves and the purpose of htis study was to see if we could achieve a blended range of vision from distance through near without compromising on distance vision binoculalry.
Setting
The study was done bu a single surgeon, Dr Dylan Joseph, in a private clinic in South Africa. 80 cases were carafelly selected and deemed good candidates for trifocal technology. Informed consent was obtained and the process of the contralateral IOL use was detailed to the patient prior to implant.
ALl patients werre followed up by the surgeon himself at one week, and 6 weeks with a manifest reafrction personally done by the surgeon.
Methods
Standard phacoemulsification was done in all cases. Toric IOL' were used at the dicretion of the the surgeon.
Ease of injector use and preloaded IOL was noted.
1 week, and 1 month post op refraction was done by the surgeon, evaluation of both monocular and binocular unaided and best corrected distance, intermediate and near visual acuity were also performed and analyzed.
Results
Best corrected distance vision better in the majority of cases using the Gemetric IOL.
Binocular distance corrected vision excellent.
Unaided near and very near vision superior using the Gemtric plus IOL.
Intermediate vision better using the Gemetric IOL.
Range of vision from 60 cm through 30cm binocularly excellent, although Gemetric outperfroms at intermediate range, and Gemetric outperfroms at very near visual range.
Binoculalry in the majority of patients, it was noted that there is a smooth transitional visual zome from 60cm, through 50 cm, 40 cm up to 30 cm.
Scotopic symptoms are comparable to current market leading trifocal IOL's.
Conclusions
Using a combination of the Gemetric and Gemetric plus trifocal IOL's from Hoya Optics provides a complete range of vision from distance, through intermediate, near and very near.
One needs to be aware that using the Gemetric plus IOL there is a slight compromise on contrast sensitivinty and on average 1-2 letters of best corrected distance visual acuity, but binoculalry this is not experienced as a complaint by most patients.
The lenses, epsecially the Gemetric plus is inotolerant to residual astigmatism, so making sure you are correcting pre-existing corneal astigmatism with the toric version is vital.
All post operative information was collected and analyzed using the IBRA/Zubisoft platform.
