The Cumulative Dissipated Energy (Cde) Counts Of Various Types Cataracts With Multi-Level And Segment Phaco Chop Technique

Cumulative Dissipated Energy (CDE) is the amount of ultrasound energy used to remove a cataractous lens. Using less phacoemulsification energy has been shown to decrease corneal endothelial cell loss and may translate to better surgical outcomes. Multilevel chop technique known useful for any density of cataract by repositioning the chopper as well as the site of occlusion with the probe at multiple planes followed by multiple segment on nucleus. This present study aimed to calculate the CDE number of various types of cataract with multi-level and segment phaco chop technique.
Cases of cataract extraction by phacoemulsification performed by one surgeons using Centurion Vision System Phaco Machine at “dr YAP” eye hospital Yogyakarta from December 2022 until February 2023. All patient data is collected from medical record data.
This was retrospective profile of cataract surgery with total of 22 eye consecutive cases of cataract extraction by one surgeon using Centurion Vision System Phaco Machine with multi-level and segment phaco chop technique.
In this study 30 % of patient have comorbidities of retinopathy diabetic, 15 % have glaucoma and 10 % have high myopia, 20 % of patient have preoperative condition of weak zonules, 15% patient have condition of posterior capsule fibrous and 10% patient have small pupil condition. There was CDE Average from all cases is 9.41 %-seconds, Hard Cataract (>NO4) : 16.03 %-seconds, Soft Cataract (<NO4) : 8.015 %-seconds with operation time average 7.32 minute and phaco time average 4.69 minute, more than 80 % cases is hard cataract type (>NO4), 42 % cases using low CDE. 72 % of patient using low CDE (<5.0) and 28 % of patient using intermediate CDE (5.0 - 10.0) all of the patient the visual acuity became 6/6 - 6/18 on the first day after surgery.
The multi-level and segment phaco chop technique is an efficient, safe, and swift procedure for various type density and condition of cataract with good visual acuity result using only low CDE counts.
