To look for proper managemnt of introperative capuslar rent and post operative macualr edema 

female patient 44 years old catract has done bilateral catract with the centorion phaco machine , first eye had capsular rent with insertion of 3 piece iol , second eye uneventful catract surgery with implanation of single piece iol in capsular bag .

diabetic catractous both eyes with posterior capsualr rent left eye 

and uneventful surgery right eye 

both eyes developed macualr edema post operatively 

44 years old patient with uncontrolled IDDM , uncorrected vision of left eye was CF , best corrected 6/36 . surgery was done with posterior cspsular rent and insertion of 3 piece iol on the sulcus , vision imporved after 6/60 after 1 week with corneal edema and macular edema , after 1 1 month to 6/9 with the use of bromefenac and predinsolone eye drops . left eye catract surgery was perfromed later with unevenful surgery , thou macular edema occured and treated with same managment 

1- careful preoperative evaluation is vital for identyfying cases who are at high risk of intraoperative -post operative complications

2-various surgical parameters should be optimised to minimize intraoperative complications 

3- post operative macular edema should be suspected in any case with vision loss after both eventfull or uneventfull surgey
