Monocular Visual Outcomes After Cataract Surgery With An Enhanced Monofocal Intraocular Lens

Purpose

To investigate monocular visual outcomes after cataract surgery with an enhanced monofocal intraocular lens implanted bilaterally.

Setting

Hospital Clínico San Carlos. Ophthalmology Department. Madrid. Spain. 

Methods

This unicentric, prospective, and observational study included 25 right eyes of 25 patients who underwent cataract surgery with an enhanced monofocal intraocular  lens (RayOne EMV, Rayner Intraocular Lenses Limited, Worthing, UK). This lens has a non-diffractive optic, designed to induce positive spherical aberration to extend the depth of focus and to improve the visual acuity at intermediate distance compared to a monofocal lens. Monocular uncorrected distance visual acuity (UDVA) at 4 m, corrected distance visual acuity (CDVA) at 4 m, uncorrected intermediate visual acuity (UIVA) at 66 cm, and corrected intermediate visual acuity (CIVA) at 66 cm were measured at 1 and 3 months post-surgery. Monocular defocus curve was also measured.

Results

The mean age was 69.23 ± 8.09 years, and the power of the implanted IOL was 21.38 ± 2.02 diopters. Visual acuities obtained one month after surgery were 0.14 ± 0.20 LogMAR for UDVA, 0.00 ± 0.09 for CDVA, 0.19 ± 0.11 for UIVA, and 0.26 ± 0.13 for CIVA. At the three-month follow-up, the visual acuities were similar to those measured 60 days earlier, with UDVA of 0.07 ± 0.15, CDVA of -0.01 ± 0.10, UIVA of 0.19 ± 0.08, and CIVA of 0.29 ± 0.08. 88% of the patients had UDVA equal to or better than 0.2 LogMAR three months after surgery. The monocular defocus curves under photopic conditions showed mean values better than 0.3 LogMAR between +1.00 D and -1.00 D. No serious adverse events were reported during the study.

Conclusions

The RayOne EMV intraocular lens showed an excellent far and intermediate visual acuity, higher than expected results for a standard monofocal one.
