Visual Results And Fixation Stability After Implantation Of An Extended Macular Vision Iol In A Series Of Amd Cases

Purpose
To assess visual acuity changes and fixation stability in advanced age-related macular degeneration (AMD) patients with mild cataract after bilateral phacoemulsification with implantation of an intraocular lens for extended macular vision (EyeMax Mono).
Setting
Prospective, nonrandomized study in University Hospital “Virgen de la Arrixaca”, Murcia, Spain.
Methods
Both eyes of 15 advanced AMD patients with cataracts (LOCS III grades 1-2 nuclear cataract) were included. Patients were implanted bilaterally with EyeMax Mono IOLs (SharpView Ophthalmology), a single lens optimized for eccentric fixation at up to 10°. The IOL power was selected to render each eye hyperopic +3.00D, ensuring retinal magnification with external spectacles. Preoperative and one-month post-operative uncorrected and best corrected visual acuity for near (33 cm) and far, monocular and binocular, were assessed. Fixation stability in twelve patients were also evaluated preoperatively and 1 month postoperatively with microperimetry to study the stability of their fixation.
Results
At one month, mean monocular CDVA and CNVA improved by logMAR 0.3 (34.5%) and 0.2 (23.3%), respectively. Binocular CDVA and CNVA improved by 0.28 (43.8%) and 0.21 (31.3%). Mean postoperative spherical equivalent was +3.00±0.50 D. Patients with preoperative bivariate contour ellipse area (BCEA) 63% > 15 deg2 or BCEA 95% > 150 deg2 did not improve postoperatively (p<0.05). No complications were detected.
Conclusions
In advanced AMD with low cataract, VA improved after EyeMax Mono IOL implantation. The IOL was found to be safe and effective. Binocular improvement occurred in all patients except one. Testing fixation stability with microperimetry allows visual prognosis prior to surgery.
