the value of crystalline lens surgery in treatment of primary angle closure glaucoma is well established. Secondary glaucoma after intraocular lens (IOL) dislocation may be combined with angle closure but needs different treatment. The purpose of the study is to present the treatment of secondary to IOL dislocation glaucoma with angle closure.

two patients (men, 82 and 84 years old), presented with red, painful eye, on topical treatment with Coll Isopto-carpine 2% X4, Coll Alphagan X2 και PO treatment with Tabl Acetazolamide 1X3. Biomicroscopy revealed anterior chamber of variable depth, angle closure in one patient and uniformly shallow anterior chamber in the second patient. Both patients had in the bag posterior chamber IOL, pseudoexfoliation and intraocular pressure (IOP) of 40mmHg and 35mmHg respectively. Visual Acuity was Hand Movements and Counting Fingers. The patients were commenced with coll Atropine and underwent IOL repositioning with fixation of the haptics by transscleral suturing.
the IOP dropped after treatment with coll Atropine. The IOP was normalized after IOL repositioning. Acetazolamide tbs were stopped after 20 and 15 days respectively and VA improved to 0.5 and 0.3 logMAR one month after surgery.
the identification and restoration of mechanical causative factors is essential for the treatment of IOL induced secondary glaucoma.
