Managing Penetrating Keratoplasty With A 4-Zone Scleral Lens Featuring Auto-Flex Technology : A Case Report

To report a scleral contact lens fitting in a patient with bilateral penetrating keratoplasty(PKP)

58-year-old patient, female, active at work; Referred from the cornea department for special contact lens fitting. As antecedents prior to fitting, the patient underwent surgery on the left eye in 1993 and in 1997 with a penetrating keratoplasty. The patient did not tolerate the use of gas permeable corneal contact lenses well. After several years of periodic controls, the patient was referred for the adaptation of a scleral contact lens in both eyes. The best visual acuity obtained without correction was 20/100 in the RE and 20/63 in the LE. The patient also had poor visual quality, with high aberrometric values with a coma of 2.58 and 2.18 respectively. After discussing with the patient the lens options for his treatment, it was decided to fit a contact lens of scleral support. The chosen scleral contact lens was ICD FlexFit.The ICD FLEXFIT is a unique scleral lens system offering an all-in-one solution for both irregular and normal corneas.Available in both 16.3mm and 14.8mm diameters. The ICD FLEXFIT is designed as a 4-Zone lens featuring Auto-FLEX technology to easily make increment adjustments to the vault or landing, while auto-adjusting the sagittal depth exactly to your patient’s cornea.The results obtained were a visual acuity of 20/30 in the RE and 20/25 in the LE, and the patient also reported an increase in his visual quality, in addition to good comfort in wearing the lens.

Scleral contact lenses represent a very good solution for patients with irregular cornea, since they provide good wearing comfort and an improvement in vision and visual quality.
